Assessment Forms

These assessment forms identify the data collected for different funding sources and project types. Data may be collected when a client enters a project (entry), at some time when they are still in the project (update), and when they leave (exit). For projects that serve clients for a year or longer, an annual assessment is required.

New Project Setup - For Existing Agencies Only

Both these documents got together for an agency who is requesting a new project to be set up in HCIS. The first one is instructions on how to complete the worksheet. This is applicable only for our existing agencies, any new agency will have to start with Agency Participation and Business Associate Agreement.
