These assessment forms identify the data collected for different funding sources and project types. Data may be collected when a client enters a project (entry), at some time when they are still in the project (update), and when they leave (exit). For projects that serve clients for a year or longer, an annual assessment is required.
HUD - This assessment is the standard assessment that is used for most HCIS projects

Project Setup
New User Setup
New users must sign the user code of ethics form for HCIS users, and the HCIS team will need the user setup form completed.
